Prescriptions are an incredibly important part of your physical, mental and overall health. Our Oak Pharmacists are here to not only prepare your medications accurately when you need them, but also to ensure your medication is appropriate and safe for you. 

Pharmacists are the experts on your medication, and we use this knowledge to inform you on why, how and when to take your medication, possible side effects, assess for interactions, and discuss the impact and efficacy of the medication on your medical conditions. 

Licensed and regulated by the Ontario College of Pharmacists, our pharmacists are highly trained and continue to receive regular education and training to remain current in our field. 

Hospital Discharges

Being discharged from hospital can be a challenging time, and your Pharmacist is here to help with making sure the process is smooth. Coordinating your medications from hospital to your home, any medication changes or health changes, and ensuring you have what you need when you’re discharged to be healthy and comfortable at home is our top priority.

Pharmacists are experts in medication and medication management, and our Pharmacists are an excellent resource for information on medications, proper doses, disease management, signs and symptoms, adverse reactions and drug interactions. 

Our Pharmacists will provide education to you on how to take medication, side effects, effectiveness and evidence, overall disease management, interactions, instruction on how to use special devices such as glucose monitors, inhalers or injectable medications and more! 

Medication Consultation

Our Pharmacists will meet with you one-on-one to assess all of your prescription medications, over-the-counter medications, your medical conditions and can provide a care plan and recommendations to you and your doctor. 

Medication reviews are incredibly beneficial to continue to ensure we have all the most current information related to your medication, medical conditions, vitamins and supplements, lab work, etc. The Pharmacist will answer any questions you may have, provide advice or recommendations, and can work with your doctor or nurse practitioner as required.

Vitamin and Supplement Consults 

Pharmacists are knowledgeable in the right vitamins and supplements that you should be taking to help with optimizing your health. Whether you’re younger, older, pregnant, taking medications that deplete vitamins or just want to know what has evidence for improving your health, our Pharmacists will provide consultation to provide you with a plan.

Pharmacy Care Clinic

What is a Pharmacy Care Clinic

In 2023 the scope of practice for a Pharmacist expanded to include the ability to assess and treat some common ailments. A Pharmacy Care Clinic is a clinic run by a Pharmacist! The Pharmacist will assess the ailment the patient presents, and make recommendations. If appropriate the recommendations could include a prescription medication! Then you have the choice to fill the script on site with us. The Pharmacy Care Clinic is a fast and easy method for our community to access healthcare! 

How Do I Access the Clinic

The clinic is operated both by appointment and walk in! The hours of the clinic are the operational hours of our pharmacy sites. Please bring your Ontario Health Card, an up to date medication list and your insurance card if you choose to fill your prescription with us. You do not need to be our patient to access this service!

When someone needs to take a blood thinning medication, it is important to monitor the INR to ensure the medication dosage is correct for the person’s medical condition. Our Pharmacists can do a finger-poke INR test in the pharmacy, and work with your doctor to ensure your medication is at the correct dosage.
