About the Role
Position Overview
We are currently seeking a compassionate, motivated, and team-oriented Pharmacist to join our dynamic Oak Pharmacy team. The ideal candidate is passionate about patient care, values collaboration, and is excited to be part of a locally owned, progressive healthcare organization.
Key Responsibilities
- Exceptional customer service and care provided to each patient
- All standard dispensing activities, including filling prescriptions, checking, etc..
- Provide counselling on prescriptions, OTC products, devices, etc.
- Ensure accuracy and appropriateness of all prescriptions dispensed
- Communicate with other healthcare professionals within the circle of care to provide customer care
- Conducting clinical services such as medication reviews, minor ailment prescribing, pharmaceutical opinions, and other clinical programs
- Developing and implementing care plans, with follow-up and documentation
- Administering vaccines
- Monitoring medication adherence
- Gathering and documenting pertinent patient, drug, and disease information
- Providing recommendations on over-the-counter products and vitamins
- Addressing patient inquiries that require clinical expertise or assessment
- Supervising pharmacy students or provisional pharmacists
- Provide exceptional patient care through professional and clinical pharmacy services
- Support workflow, mentorship, and team development
- Help foster a positive patient experience and welcoming pharmacy environment

Requirements
Qualifications
Licensed and in good standing with the Ontario College of Pharmacists (OCP)
Authorization to administer injections in Ontario
Prior experience in community pharmacy is an asset
Strong communication, organizational, and interpersonal skills
A passion for patient-focused care and community health
Ability to adapt in a fast-paced, collaborative environment
